Root causes of the incidents. One can edit the pre-set causes (put the cursor in the cause field, add changes and click the pen symbol), add new causes (enter the name and click the plus symbol), delete a cause (put the cursor in the cause field and click the minus symbol):
Edit, delete or add new criteria in the field “What effect this situation had on your organization”, same as with the field “Root causes of the incident”.
